To ensure the health and safety of our campus community, the University is working to help all employees and designated campus colleagues (DCCs) get vaccinated against COVID-19 as soon as possible; you can help by completing a voluntary short survey by Tuesday, Feb. 9 at 6 p.m.
All UArizona faculty and staff are encouraged to take the survey to help inform the university about the progress of vaccine rollout among our campus community, specifically how many employees have received vaccinations, how may are opting out and how many are having trouble making appointments.
About the survey
- The voluntary short survey will take just a couple of minutes to complete.
- You will be asked for your NetID but personal identifiable information will be used only in cases where a respondent has reported barriers so that the university can follow up to assist.
- Please note that your responses to this survey will not be shared with your supervisors and that neither your vaccination status nor your participation in this survey will be used to dictate your work location.
- We may ask you to complete the survey again in the coming weeks and months as we continue to monitor vaccination participation and barriers. The aggregated results of the initial survey and later surveys be shared with the campus community.
