Advances in Aging – Using Patient Priorities Care as a Tool to Provide Patient Centered Care

Dr. Fonseca-Valencia is a geriatrician and clinician educator with special interest in caring for complex older adults of underserved and multicultural backgrounds. After completing her training in geriatric medicine at the Harvard Medical School (HMS) multicampus fellowship program and the New England Geriatrics Research Education and Clinical Center (NE-GRECC) she joined the faculty at Beth Israel Deaconess Medical Center (BIDMC) and HMS, where she provides direct clinical care, supervises trainees (including geriatrics fellows, internal medicine residents and HMS students), and directs the HMS multicampus geriatrics fellowship BIDMC site as well as the Division of Gerontology consult service at BIDMC, with the goal of enhancing the care provided to older adult patients.

  • Discuss the challenges and opportunities faced when providing person-centered care
  • Identify Patient Priorities Care as a communication model used to help guide the care we provide our patients
  • Demonstrate the applicability of the Patient Priorities Care model in primary care settings
