Join Arizona Telemedicine Program, Southwest Telehealth Resource Center and the American Indian Health – Area Health Education Center for a session focusing on how health care professionals can engage patients in conversations on advance care planning, and how the Arizona Advance Directives Registry can make sure their wishes are recorded and available to all their providers.
